Eldercare. In Home Support
 

The Community Alternative System Agency (CASA) is a partnership between Jewish Family Service (JFS) and the Erie County Department of Social Services. It is designed to help the frail elderly and disabled children and adults remain in the most appropriate, least restrictive setting, utilizing all available community resources, informal supports, and formal home care services.

There is a two-step process for enrollment in home long-term care: an Erie County caseworker determines financial eligibility, and the JFS / CASA staff determines physical eligibility.

Once a family or individual is qualified for service, the CASA staff and Erie County caseworker facilitate access to a complement of Medicaid home care and other support services. Our goal is to provide quality, cost-effective long-term care based on individual need.

Additional Services

JFS / CASA staff also advises individuals and families on ways in which to make the home safe, and assists in developing a new care plan if home care is no longer an option or safe option. We also provide referrals to other services within Jewish Family Service or to other community providers.
