JEWISH FAMILY SERVICE OF BUFFALO & ERIE COUNTY NEW YORK

Home-based assessment & therapy for seniors
with mental health concerns

  • Brief psychotherapy
  • Assessment and diagnosis
  • Medication recommendations
  • Linkages to case management and social day programs
  • Depression, anxiety, and psychotic disorders

Geropsychiatric Assessment Program (GAP) Is About
Preventing hospitalizations that occur as a result of untreated mental health problems in seniors.
Project GAP reaches out to a population which generally does not access outpatient psychotherapy services. A New York State licensed psychologist diagnoses the senior after conducting an at-home or in-clinic psychological interview. Therapy is then offered on a biweekly basis. When necessary, the client is linked to additional services. These may be for medication, case management, and social day programs.

Eligibility Is Determined By
Age and the suspicion of a mental health disorder
You must be 60 years of age or older, homebound, and have the symptoms of a mental health disorder such as depression, anxiety, or a psychotic disorder. We cannot work with clients who have an organic mental health disorder such as dementia or substance abuse disorder as a primary diagnosis. As with all our services, GAP serves residents of Erie County, regardless of sex, race, or religious orientation.

Our Goals Include
Stabilizing acute psychiatric symptoms and providing linkage/referrals to community services
GAP provides rapid assessment and brief psychotherapy (10-20 sessions) services in the client's home. We endeavor to combine mental health treatments, case management and social day programs to meet each individual's needs. Typically, by combining psychotherapy and appropriate linkages, clients improve within short periods of time.

Fees Are
Generally covered by Medicaid, Medicare and most private insurance.
A sliding fee scale is available to those who do not have insurance. Individuals are eligible for services, regardless of ability to pay.

Gap Is Located
And provided in the privacy of the individual's own home.
Our clinical staff is headquartered in our 70 Barker Street office in Buffalo, but assessment and therapy is done in the client's own home.
