Care Management Services

  • Self-Care assessment and planning
  • Linkage and advocacy with community providers/resources, entitlement programs, government benefits, crisis intervention and counseling

In-Home Support Services
  • Kosher home delivered meals (in conjunction with Meals-on-Wheels of Erie County)
  • Assistance with completing medical and insurance claims

Mental Health Services
  • Counseling (home or office) - Individual, marital, family, or group
  • Psychiatric consultation
  • Medication management
  • Personal enrichment workshops
  • Peer support/mutual aid groups
  • Socialization opportunities

Family Support Services
  • Caregiver consultation (local or out-of-town)
  • Resource information and referral
  • Nursing and adult home placement assistance

Jewish Family Service offers comprehensive assistance to older adults that promotes the
highest levels of independence, dignity and personal fulfillment.
